Feeding jejunostomy, a palliative surgical procedure, was planned to provide long-term enteral nutrition to the patient, who had difficulty swallowing due to tumor involvement of the esophagus and surrounding structures. We report a case of a 75-year-old male with advanced squamous cell carcinoma of the esophagus, complicated by metastases and difficult airway.This procedure involves the placement of a feeding tube into the jejunum, bypassing the stomach and allowing direct nutritional support. It is commonly used in patients with advanced malignancies, particularly those with compromised swallowing or gastrointestinal function. In this case, a combined approach of dexmedetomidine infusion, bilateral intercostal nerve block, and bilateral transversus abdominis plane (TAP) block was used to ensure adequate anesthesia and pain control during the procedure. This case highlights the importance of a tailored anesthetic approach in managing complex airway and analgesic challenges in advanced cancer patients undergoing palliative surgeries like feeding jejunostomy.

This study examines the rise of tea cafés in metropolitan India, exploring their transformative impact on traditional tea consumption and urban culture. Once synonymous with roadside stalls and domestic rituals, tea in India is now experiencing a modern evolution through curated café spaces. Brands like Chaayos, Chai Point, and boutique tea houses have popularized tea cafés as hubs that merge traditional chai culture with contemporary consumer preferences. Driving this shift are factors such as urbanization, rising disposable incomes, and a growing appetite for experiential consumption. Tea cafés appeal particularly to younger urban demographics by combining quality offerings with inviting aesthetics, diverse menus, and digital engagement. The organized tea café market, valued at ₹873 crore and growing at a CAGR of 25.1%, signifies significant momentum within India’s larger tea market, worth $11.7 billion in 2024. This research investigates the factors fuelling the popularity of tea cafés, from branding and ambiance to evolving consumer behaviors. It also analyzes the demographic profile of urban patrons, their preferences, and lifestyle trends. The study further examines the challenges and opportunities for sustainable growth in this segment, providing insights into how tea cafés are redefining tea culture in India's metropolitan landscapes while balancing tradition and innovation.By delving into these dynamics, the study aims to uncover the interplay of modernity and heritage shaping the future of tea consumption in India’s urban spheres.

Today the word democratic has loosed its popularity within the people from some particular state. Every utterance of human beings is a part of some narratives. The human existence itself is a narrative that hides many stories. The identity of each person is a narrative. For instance, the gender of any person contains a narrative that includes so many facets of that particular narrative created over a while. However, narrative creation is not an innocent practice. The creation of any narrative depends on who creates the narrative. Thinking and rethinking narratives is a never-ending practice. As long as man lives on the earth and can think, he will come up with different narratives. The historical renderings of various events come to us through stories. The historian cooks the raw factual details and converts them into the recipe of a story. He weaves them into the background of his social, political, and cultural lineage. In recent years different subaltern groups have emerged with narratives and discourses that challenge the dominant narratives. Adivasis of India are one among them. The oral narratives of the Adivasis were never regarded as of any worth. However, in the previous few decades, Adivasi intellectuals have emerged and are making their stories heard not only in India but throughout the world. Their worldwide recognition as representatives of Indigenous communities is testimony to their strong resisting voices that challenge the mainstream narratives. They are re(thinking) narratives regarding them which the mainstream narratives have distorted. Thinking and rethinking of Adivasi discourse is both destructive and constructive. This paper attempts to understand Adivasi identity narratives in the light of poems written by Adivasi poets and how they challenge the hegemony of mainstream narratives.

Food security is a pressing challenge in urban India, particularly in economically vulnerable areas. The Global Hunger Index 2023 ranks India 111th out of 125 countries, highlighting significant food insecurity issues. This paper explores the theoretical foundations and socio-economic consequences of rising urban food insecurity, with a focus on marginalized communities. Through a case study of Kapashera, a low-income locality in Southwest Delhi, we assess the effectiveness of the Public Distribution System (PDS) in alleviating food insecurity. Our research employs primary data from fieldwork, including interviews and participant observations, alongside secondary sources to contextualize the findings within broader national PDS dynamics. The study highlights residents' grievances, this paper seeks to contribute to a broader understanding of the socio-economic implications of neoliberal reforms in India's PDS. The analysis underscores the urgent need to critically examine the impact of these reforms on the most vulnerable populations and to recognize that it's high time for the state to focus on its citizens rather than weakening them through bureaucratic hurdles, surveillance and other restrictive measures.

Making decisions is a common idea. Every step along the way, young adults make decisions about their careers, romantic relationships, marriages, and families. Making decisions involves deciding a course of action to take from a range of options. The purpose of this study is to conclude on the several aspects that influence young adults' decision-making. Given this, the current research assesses the impact of multiple aspects and offers recommendations for improved decision-making. This research will yield scholarly insights, as decision making errors are expensive and getting more expensive, it is necessary to concentrate emphasis on the search for elements that affect Decision-making. The researcher focuses on literature review to come up with the key factors of Decision-making. Background of the study: Pre-diabetes is an intermediate state of hyperglycemia with glycemic parameters above normal level but below the Diabetes threshold. It is considered to be at risk state, with high chances of developing Diabetes. Following Pathya becomes essential in this stage of the disease. Masura Yusha has been referred as "Pramehahara" in Yogaratnakara and Kaiyyadeva Nighantu. Objective: To evaluate the effect of Masura Yusha as Pathya in Pre-diabetes Methodology: A minimum of 60 patients fulfilling the inclusion, exclusion and diagnostic criteria of Pre-diabetes were selected and randomly divided into two equal groups, group A and group B. 30 patients of group A were advised with dietary intervention and 30 patients of group B were advised with Masura Yusha and dietary intervention for a period of 15 days. The outcome measures were assessed at baseline, on day 15th, 30th, 45th day for Increased thirst, Frequent desire to urinate, FBS and PPBS. Result: As per the results, the test shows statistically significant results in both the groups (group A and group B) with group B (Masura yusha with Dietary Intervention) having better result than group A (Dietary Intervention) in controlling Increased thirst, Frequent desire to urinate, FBS, PPBS. Conclusion: Masura Yusha has better result than Dietary Interventions in Pre-diabetes.
